On iOS why is the fare restricted again when I've already purchased the Unrestricted Fare in-app item?

Following a recent app update, several customers have reported that their Unrestricted Fare in-app purchase is missing, so the fare is once again restricted to $10.  App updates are usually a seamless process with in-app purchases being automatically restored by Apple but clearly something has gone wrong on this occasion.

To manually restore the in-app purchase, go to the Menu > Upgrade page and press the Restore button, see screenshot below.
